Axis Health System offers the following resources about COVID-19, or coronavirus, to support our community members.

The information on this page will help you understand the process required to get your Axis Health System health records and the forms to make your request. Axis Health System is an integrated healthcare provider, which means your records may include medical care, behavioral healthcare, substance-use treatment or oral healthcare. Requests for all information (including progress notes, consent forms, lab results, etc.) can delay processing, become very costly and can sometimes be much more information than you really need. You can request your health records once a year for free. Subsequent requests require a health records fee. Axis Health System will charge for the reasonable costs of providing your records. Click for a list of Health Records Fees that will apply.
