Community Health Center
Cortez Oral Health Clinic is one of Axis Health System's five federally designated Community Health Center (CHC) sites. CHCs are located in medically underserved areas throughout the nation. CHCs make comprehensive primary care accessible to all people regardless of insurance status and improve the health of the whole community. Cortez Oral Health Clinic is supported by the Health Resources and Services Administration (HRSA) of the U.S. Department of Health and Human Services (HHS) as part of an award totaling $2,258,803 or 35.6% of the total operating funds for the three sites financed with non-governmental sources.

We welcome all ages and encourage early care for children – starting with the first tooth. This clinic is a key resource for adults with Medicaid or who do not have dental insurance.
Now accepting new patients
We welcome new patients, and offer a variety of payment options. We accept Medicaid, CHP+ and people without dental insurance, and offer a sliding-fee scale for those who qualify. We accept all patients at the AHS Oral Health Clinic, but do not accept dental insurance at this time. If you would like to receive care, and you do have dental insurance, we will collect the charge from you and then we will give you what you need to file for reimbursement from your insurance company.
