I thought Axis only offered mental health services. Do they offer medical care now? 

Axis Health System has offered medical care since 2011. Four Axis clinics offer a combination of integrated services (mental health, primary care and dental) all under the same roof. The four clinics are: La Plata Integrated Healthcare, Cortez Integrated Healthcare, Archuleta Integrated Healthcare and Dove Creek Integrated Healthcare

Are Axis clinics offering COVID-19 testing?
Four Axis clinics are currently offering testing: La Plata Integrated Healthcare in Durango, Cortez Integrated Healthcare, Archuleta Integrated Healthcare and Dove Creek Integrated Healthcare. Same-day appointments are often available.

Do you have to be an established Axis patient to get tested?
Yes, you must be a current Axis patient to receive a COVID-19 test. All of our clinics are accepting new patients. If you are not an established patient, click a link below for other community testing resources.

How do I sign up for a vaccination through Axis?
Current patients may sign up for a COVID-19 vaccination by filling out our Online Vaccination Scheduling Form.

Can Axis clinics do rapid COVID-19 testing?
A rapid test is available to established patients who are symptomatic when supplies are available.

Does Axis offer COVID-19 vaccinations? 
COVID-19 vaccinations are available by appointment for Axis patients. First dose, second dose and supplemental doses for immunocompromised individuals are available by appointment at Archuleta Integrated Healthcare, Cortez Integrated Healthcare and La Plata Integrated Healthcare.

Are COVID-19 vaccination booster shots available at Axis clinics? 
Information on booster shots is changing rapidly. For the latest eligibility information, visit this CDC page As booster shots for each vaccine are authorized and become available, we will notify eligible patients who originally received a COVID-19 vaccination through Axis.

Which COVID-19 vaccine does Axis offer?
As of early October 2021, Axis clinics are currently offering Pfizer only.
