General Medical Evaluations

- Fever without known cause
- Mild asthma or wheezing

- Dehydration (mild to moderate)
- Urinary tract infections (UTIs)
- Mild abdominal pain (non-emergency)
- Minor allergic reactions or hives

Minor Injuries & Wound Care
- Sprains and strains
- Fractures & dislocations (non-life threatening)
- Cuts, lacerations, and abrasions
- Minor burns (1st or mild 2nd degree)
- Blisters, boils, abscesses

Diagnostics & Testing
- X-rays (chest, ribs, limbs, etc.)
- ECG/EKG
- Rapid flu, strep, RSV testing
- COVID-19 antigen & PCR testing
- Urinalysis and pregnancy tests
- Blood glucose and hemoglobin
- STD testing (HIV, chlamydia, gonorrhea, syphilis)
- TB skin test or IGRA
- Point-of-care CBC or BMP

Care coordination is when someone helps you pull together the pieces of healthcare. This puzzle fits your unique needs. The people who provide care coordination are called Community Health Workers. They act as your personal guide in a number of ways. First, they help you find what you need in the healthcare system. They match you to providers and make sure your needs and preferences are met. Second, they coach you to create and follow through on your health goals. Finally, they work to lower healthcare costs for people on Medicaid and Medicare.
When to Call 911
For serious emergencies, you should act quickly. If you have chest pain, a bad injury, or trouble breathing, call 911 or go to the nearest emergency department or hospital. If you have an after-hours medical concern (that is not a medical emergency), call 970.247.5245 to talk with a Registered Nurse (RN).
