Pagosa Springs, CO – Axis Health System (Axis) and Pagosa Medical Group (PMG) are pleased to announce that PMG will become a part of Axis on August 4, 2025. This partnership aims to strengthen healthcare services currently offered independently by both organizations.
Key Details:
- The PMG Urgent Care clinic is a part of this change but will continue to operate at the current location for the foreseeable future with no change in hours of operation.
- The PMG Primary Care clinic will continue to be available at the current location for the time being, as part of this change. These services are expected to be relocated the Axis Pagosa Springs Integrated Healthcare Clinic in the fall.
- The current services available at the Axis Pagosa Springs Integrated Healthcare Clinic, which include primary care, oral health and behavioral health, will continue to be available without change.
This consolidation of resources and talent is designed to improve access and care delivery by optimizing the infrastructure and resources that Axis and PMG have independently created.

Patients will continue to see the same providers and team they’ve come to know and trust. More information will be shared directly with patients through the patient portal and by mail.
