Axis Announces Closure of Norwood Clinic Location

Care will remain available through telehealth and other Axis locations

NORWOOD, Colo. — Axis Health System’s Norwood clinic lease will end effective October 31, 2026, and the lease will not be renewed, nor will another physical clinic location be established in Norwood at this time. The decision follows careful consideration as Axis works to maintain healthcare services throughout the region.

Axis recognizes that the closure may create challenges for patients who rely on care close to home. Depending on their healthcare needs and service availability, patients may receive care through telehealth or in person at another Axis location, including Montrose or Cortez.

Questions about your care?

Call 970.252.3200 with any questions and Axis staff can explain available options and assist with scheduling and care coordination.

Medical records questions?

Medical records will remain securely maintained by Axis. Patients continuing care within Axis do not need to transfer their records. Those choosing a provider outside Axis can request assistance with transferring their records.

Axis appreciates the trust of Norwood patients and remains available to support them through this transition.
