Veterans Outreach
Axis has worked to expand access to high-quality, integrated care for Veterans and Military Families across Southwest Colorado through the creation of a dedicated Veterans Liaison position, funded by the SAMHSA Certified Community Behavioral Health Clinic (CCBHC) grant. The position was developed to strengthen outreach, coordination, and engagement with Veterans and their families, ensuring timely access to behavioral health, primary care, and essential support services. By partnering with local Veterans Service Offices, the VA, and community-based organizations, the Liaison streamlines care coordination, bridges gaps between systems, and connects individuals to benefits and resources that promote both physical and mental well-being.
Since the launch of this initiative, Axis has seen a 30% increase in Veterans served from FY24 to FY25, reflecting the impact of this targeted and relationship-based approach. Tammy Jivery, who serves as Axis’ Veterans Liaison, is the daughter of a Purple Heart recipient who served in combat in Vietnam. Her father’s experiences, living with PTSD, a substance use disorder, and navigating complex systems, shaped her deep understanding of the challenges faced by Veterans and their families. His service, sacrifice, and struggle continue to inspire her work today. Tammy approaches each interaction with empathy, respect, and determination to ensure that Veterans receive the coordinated, compassionate care they deserve.
