Axis Health System (Axis) is soliciting competitive proposals from qualified vendors to complete the year three Updated Community Needs Assessment (CNA) required for Axis’s Certified Community Behavioral Health Clinic (CCBHC) grant requirements to SAMHSA. Per SAMHSA guidance, CCBHCs seeking certification or re-certification should refresh their CNA at least every three years, or sooner if community conditions change significantly. This procurement adheres to SAMHSA and HRSA procurement standards, as well as 45 CFR §75.327–335.
Responses to RFP Questions
A. Axis will provide a list of appropriate stakeholders and contact information. Can also support outreach. Axis will determine the outreach plan with the consultant and the best approach.
Q. The RFP indicates that the consultant should “attend project meetings.” Does Axis have standing meetings or would these be meetings the consultant and Axis set up together?
A. These would be determined by the consultant and Axis based on project deliverables and needs as they arise.
Q. Does Axis have a required conflict of interest disclosure form, or would a statement from the applicant suffice?
A. A statement would be appropriate.
Q. The RFP does not specify a specific number of references. How many would you like included?
A. Two (2).
Q. The RFP indicates that the draft report due 3/1/25 and the final report is due 3/15/25. I assume this is a typo and should be 26. Would Axis commit to providing feedback on the draft report by 3/8/26?
A. Yes, this is a typo. Draft would be due 3/1/2026. Axis would provide feedback by 3/8/26.
Q. The RFP indicates a fixed price of $20,000. Previously, indications had been $25,000. Please confirm the maximum allowable budget.
A. The budget is $20,000 as outlined in the CCBHC budget. If this is not reasonable, please indicate what would be included with an additional $5,000.
