Get your health records

The information on this page will help you understand the process required to get your Axis Health System health records and the forms to make your request. 

Axis Health System is an integrated healthcare provider. Your records may include medical, mental health, substance-use treatment or dental care. Requests for all information (including progress notes, consent forms, lab results, etc.) can delay processing, can sometimes be much more information than you really need and may include a fee. You or your legal representative have the right to your health records. Submit all questions, requests or forms to Axis Health System HIM Department at HIM@axishealthsystem.org, via your Patient Portal, fax 970.247.7885 or mail 185 Suttle St., Durango, CO  81303.

*The portal is only for integrated patients who have registered for the portal, you cannot self-register. To register, call or visit your Axis Integrated Healthcare clinic.

Options to access health records

Patient Portal – If you are 18 or older and a patient at one of our integrated clinics, you may access some of your electronic health records via your Patient Portal. You may request copies of your full health record, download and share records, or grant friends/family proxy access to your portal by completing the Patient Authorization for Third Party Access to Patient Portal Form. All medical information that is made available on your Patient Portal will also be made available to your proxy. Don’t have a portal? Submit your request to HIM@axishealthsystem.org.

Electronically (email or fax) – Axis Health System gives you or your personal/legal representative access to your electronic health information in an electronic format, free of charge and without delay, per the 21st Century Cures Act Final Rule effective April 5, 2021.

Paper – If you need hard copies of your records, you will be charged fees established by HITECH and the State of Colorado. Click for a list of Health Records Fees that will apply. Complete the Patient Request to Access Health Records Form.

Accessing minor patient health records – Minor children (ages 12-17) who are patients at our integrated clinics and their parents/legal guardians may have access to their electronic health records via their Patient Portal when both parties give permission by completing the Authorization to Disclose Minor Patient (Age12-17) Electronic Health Information to Parent/Guardian or Minor via Patient Portal Form. Access to a Patient Portal can be revoked by the patient, parent or legal representative at any time by submitting this request in writing to the Axis Health System HIM Department at HIM@axishealthsystem.org, via your patient portal, fax 970.247.7885 or mail to 185 Suttle St., Durango, CO  81303.

Authorize someone else to obtain copies of my health records – Axis Health System may release your health records to a third party initiating a request for PHI on its own behalf with a HIPAA-compliant authorization and upon payment of reasonable fees. Amend your health records – If you think the information in your health record is incorrect, you can request a change to your record with the Amend Your Protected Health Information Form.
