The information on this page will help you understand the process required to get your Axis Health System health records and the forms to make your request. You or your legal representative have the right to your health records. Find out about your rights and more from the Office of the National Coordinator for Health Information Technology.

To fully understand your healthcare privacy rights at Axis Health System, you can also read our Notice of Privacy Practices.

Axis Health System is an integrated healthcare provider, which means your records may include medical care, behavioral healthcare, substance-use treatment or oral healthcare. Requests for all information (including progress notes, consent forms, lab results, etc.) can delay processing, become very costly and can sometimes be much more information than you really need. If you need help deciding what to request, please call 970.828.2526

You can request your health records once a year for free. Subsequent requests require a health records fee. Axis Health System will charge for the reasonable costs of providing your records. Click for a list of Health Records Fees that will apply.

If you think the information in your health record is incorrect, you can request a change, or amendment, to your record. Click to view and print Amend Your Protected Health Information Form.
