Axis is ready to assist you in improving your health and quality of life. We have a wide range of programs to help people address their unique needs and live life to the fullest.  

Identifying needs 

When you make the call to Axis, one of our team members will ask several questions to gather some basic information. A trained staff will follow up with you to collect information to help pair you with the right provider for your symptoms.  The questions will include asking for a brief overview of your needs, if you have any treatment goals, if you have any previous treatment experience, document any medications you may be taking, and identify any barriers you may have in arriving for an intake appointment.  

At the intake appointment, the clinician will get to know you, your concerns, and your health goals. To better understand your symptoms and needs, you’ll complete any appropriate screening questionnaires (which are very brief), and you and your Axis clinician will develop a treatment plan and explore options that are right for you. This treatment plan may include different services tailored to address your needs. Treatment plans can include specific therapy programs, medication, or other supports or a combination, thereof. Treatment plans can also incorporate resources about housing, education, job skills training, and other resources and coordination if needed.    

Depending on the treatment plan, most people will engage with multiple Axis team members. Your team may include therapists with different specialties, case managers, or other providers who can help you achieve your health goals. Some people may only need services for a short time to help them manage a specific life event or circumstance and others may work with their care team longer as they continue their path to recovery and wellness.  Sometimes, a person’s needs will change during a course of treatment. Your care team will work with you to adjust your treatment plan as needed.   

Axis accepts most insurance, including Medicaid, Medicare, and private insurance. In addition, we offer an income based sliding fee discount program to help those who qualify.
